 Filling in large holes and building up bases.  Not leveling yards, not finish grading around house, not completing jobs by hand.  

#2) What should fill dirt look like?

Fill dirt looks like a dense, pale, and rocky mixture of clay, sand, and silt.  Large clumps possible with rocks (up to softball size) and roots

#3) Is Fill Dirt processed?  No, fill dirt runs a wide range of anything dug up from the ground containing natural and unnatural products.
